Chamber to co-sponsor health forum on Nov. 30

The Amherst Chamber of Commerce, in conjunction with Independent Health, will present a forum for its members at 8:30 a.m. Tuesday, Nov. 30, at its office, 350 Essjay Road, Suite 200, Williamsville.

The topic of the forum will be the Patient-Centered Medical Home pilot program.

The focus of the PCMH initiative is to redirect individuals back to building meaningful relationships with their primary care physicians.

In turn, physician practices are redesigning their systems of care, developing new collaborative relationships with other providers and using new electronic technology to improve the delivery of care.

Independent Health representatives Dr. Judith A. Feld and registered nurse Kim Fecher will discuss the pilot program, how it was implemented and the kind of results being reported so far among participating physicians in Western New York.

Feld is the associate medical director of behavioral health, while Fecher is the director of practice management and physician services.
